When you download your CLE Attendance Form for an LACS event that you attended, please don't forget to also download a copy of the LACS Evaluation Form.  Kindly fill-in the form regarding the event and send it by email to  marie.varricchio-diaz@fox.com   Your thoughts about our programs are important to us in planning future events and we need your completed evaluation forms to help maintain our status as an authorized MCLE provider.  Thank you for your help!
